Patient Enrollment is simple, secure, and flexible

Our inter-operable, HIPAA-compliant platform streamlines workflow and enrollment across multiple sites and integrates seamlessly into EHR or 3rd party referral platforms.

Your team identifies eligible patients

and sends their data to our platform.

You set the monthly disbursement amount

program duration, and eligible healthy food that fits your patient’s care plan. Once enrolled, a Fresh Connect Prepaid Card is automatically mailed to the new cardholder. After activation, the card is ready to use and reloaded monthly.
